REQUIREMENTS

• All applicants must be present during application process

• Both parents must be present for applicants under the age of 16

• Proof of Citizenship (i.e. original birth certificate issued by the State or County – abstract of birth or hospital birth certificates are not acceptable, original naturalization certificate, previously issued passport)

• Valid Government issued identification (i.e. drivers license, state issued ID) AND a copy of the front and back of the ID.

Ų Photocopies must be ONLY on 8-1/2 x 11" standard white paper and must contain images on only one-side of each page. Applicant may present two separate pages - one page displaying the front of the ID and the second displaying the back of the ID.

Ų The applicant can choose to enlarge (i.e. copy at 200% size) the image of the ID but cannot decrease the size of the image of the ID.

Ų Applications of minors must be accompanied with photo copies of IDs of both parents.
