The City of Anaheim has resumed
Passport Acceptance Services by APPOINTMENT ONLY.
Appointments are available from 9:00am - 3:00pm,
Monday through Friday
PLEASE READ ALL INFORMATION AND INSTRUCTIONS ON THIS PAGE
AND SCHEDULE YOUR APPOINTMENT BELOW.
To schedule an appointment, select an available date and time for an appointment for Passport Services. Each appointment requested is for one applicant. Complete additional appointments individually for multiple applicants as needed. For additional assistance, please call 714-765-4540.

IMPORTANT: Please review all requirements and the fees below prior to your scheduled appointment. If you are missing any of the required items/documents or required payments at the time of your appointment, you may be asked to reschedule your appointment at a later date. Please call 714-765-4540 with any questions or with any extraordinary situations you may have. Plan to arrive 15 minutes prior to your scheduled appointment time to account for parking and the security screening when entering City Hall. |

Requirements
- Completed Application [unsigned application (Form DS-11) completed single-sided in black ink]:
- Social Security Number is required on the application
- Forms can be found online at https://eforms.state.gov/
- Evidence of U.S. Citizenship(one of the following):
- Certified Birth Certificate
- Minors under age 16 must submit Certified Birth Certificate to show parental relationship
- Abstract Birth Certificates nor Hospital Birth Certificates are acceptable
- Consular Report of Birth Abroad
- Certificate of Naturalization
- Previously issued undamaged U.S. Passport
- Applicable for adult applicants not eligible to renew their passport by mail
- Certified Birth Certificate
- Photo Identification(one of the following):
- Valid Driver's License
- Note: Out of State Identifications require a secondary form of identification
- Current Valid Passport
- Valid Military Identification
- Valid Driver's License
- All Applicants must be present:
- All applicants, including children, must be present during application process. For children under 16, both parents (with identification) must be present at the time of applying. If both parents are not able to appear in person, call before your appointment for additional requirements.
- Passport Photo:
- One 2" X 2" Passport Photo
- Photo Services available onsite
- Visit travel.state.gov/photos for passport photo requirements
- Payments (two separate payments, Cash not accepted) see Fees:
- Application Fee (U.S. Department of State) paid by Check or Money Order only
- Execution Fee (City of Anaheim) paid by Check, Money Order, or Visa or Mastercard
Two Separate payments are required - (1) U.S. Department of State and (2) the City of Anaheim (*Cash not accepted)
