regulations governing the proposed Special Event under the Anaheim Municipal Code. I
understand that this application is made subject to the requirements, eligibility, application
process and selection procedure established by the City Council Policy 1.5 (“Special Event Financial Support Policy”) adopted 10/06/15. I certify that I am authorized to submit this application on behalf of the Organization and to be bound by the information contained herein, and therefore, agree to be financially responsible for any costs and fees that may be incurred by or on behalf of the event to the City of Anaheim.

I further acknowledge that information contained in this application is a public record and may be subject to public inspection pursuant to the California Public Records Act (CA Gov. Code 6250 et seq.).
